You will have to inspect actual AMI events, to see if you have proper JOIN and LEAVE events for that transferred call. If there are missing events, or they channels or uniqueids do not match between join/leave, then FOP2 won't clear up those waiting calls.

Are you using supervised transfers or blind transfers? Are you using your phone transfer feature or the asterisk builtin transfer methods (features.conf)?
